India Launches SAHI: A Bold AI Strategy to Revolutionize Rural Healthcare

India has unveiled a new national strategy to integrate artificial intelligence into its healthcare system. The plan, called SAHI, was launched at the India AI Impact Summit 2026 by the Ministry of Health and Family Welfare. Its goal is to provide high-quality, affordable, and timely medical care for all, in line with the country's #AIforAll vision.

The strategy focuses on five key areas, including governance rules to ensure safe and accountable AI use at scale. It builds on existing digital infrastructure like the Ayushman Bharat Digital Mission and the BODH Platform to bring advanced diagnostics to rural regions with few specialists.

SAHI aims to expand universal health coverage by using AI-driven tools in underserved areas. Programs like Scaida BrainCT for brain scans, Nayanamritham 2.0 for eye care, and Cough Against TB (CATB) for tuberculosis detection have already shown results. However, adoption remains low in rural facilities due to poor internet, high costs, and a shortage of trained staff.

The government, working with the World Health Organisation (WHO), stressed the need for ethical oversight. Challenges include biased algorithms from limited data, distrust among clinicians, and the strain of validating large imaging workloads. To address these, the strategy calls for national investment in cybersecurity, bias reduction, and safety monitoring—all while upholding WHO's six core AI principles.

India's push for AI in healthcare follows earlier initiatives like the National Strategy for AI and the National Health Stack. These efforts lay the groundwork for scaling up tools such as SMARTON and Virtual Cardiac Twin, which simulate heart conditions. Yet, hurdles persist, from data security concerns to resistance in remote communities where technology uptake is slow.

The SAHI framework sets out a roadmap for responsible AI adoption in India's health sector. It prioritises ethical stewardship, improved infrastructure, and training to overcome current barriers. Success will depend on sustained investment and collaboration between government bodies, healthcare providers, and global partners like WHO.
