Breakthrough MANGROVE trial reveals chemotherapy-free hope for mantle cell lymphoma patients

BeOne Medicines Ltd. has announced positive results from its Phase 3 MANGROVE study. The trial compared a chemotherapy-free regimen of BRUKINSA (zanubrutinib) plus rituximab against standard chemoimmunotherapy in untreated mantle cell lymphoma (MCL) patients. MCL is a rare and aggressive form of B-cell non-Hodgkin lymphoma, often affecting older adults. The MANGROVE study is the first of its kind. It evaluates a BTK inhibitor-based, chemotherapy-free approach against the long-standing standard of bendamustine plus rituximab (BR). Chemoimmunotherapy, while effective, often causes significant side effects, including myelosuppression, prolonged immune suppression, and cumulative toxicity.

The trial met its primary goal. Patients on BRUKINSA plus rituximab showed a 43% reduction in the risk of disease progression or death compared to those on BR. Progression-free survival (PFS) improved significantly with the new regimen. Overall survival (OS) data also favoured BRUKINSA plus rituximab, though the results were not yet mature at the time of analysis.

The safety profile of the BRUKINSA combination matched the known profiles of both drugs. No unexpected adverse effects were reported. The MANGROVE trial demonstrates a potential shift in frontline MCL treatment. A chemotherapy-free regimen with BRUKINSA plus rituximab offers improved progression-free survival. The approach may reduce the burden of toxic effects associated with traditional chemoimmunotherapy.
